MyUFHealth

MyUFHealth, formerly known as MyChart, is offered by UF Health to encourage all patients to participate in their health care. This service is available to all patients who are seen in any UF Health Office or department. This online platform provides a secure and accessible way to communicate with your health care provider. It allows you to access your information anywhere on your computer or smartphone, look up your released test results or review your health history, and request prescription refills. You can also schedule an outpatient appointment and be reminded of upcoming appointments. MyUFHealth saves you time and gives you the privacy you deserve.

How to Access the Sign up Page

To download the app on your smartphone, access your app store (iPhone users) or GooglePlay (Android users). Search “MyChart” by Epic and download the app (it’s free!).

To sign up using a computer or laptop, access https://mychart.shands.org/mychartprd/.

How to Sign up

Once you have accessed the sign up page, click on “Sign up now”.

The first step will ask for your activation code. Your activation code is listed on the back of your pre-visit summary that you receive when you sign into your appointment. Your activation code should consist of a combination of letters and numbers. Enter the activation code in the boxes provided. You will not need to use this code after you sign up.

After entering the activation code, enter the patient’s zip code and date of birth. Click “Next”.

The second step will ask you to create a username and password. Your username should not consist of the patient’s first and/or last names or an email. Your password is case-sensitive and must be at least eight characters. It will ask you to enter your password twice to make sure you have typed in the correct information.

After creating a username and password, pick a security question. This question will be asked if you forget and try to retrieve your password. After you pick your security question, type in the correct answer in the space provided. Click “Next”.

The third step will ask you to confirm or provide an updated email address. Click “Next”.

This last step will ask you if the information provided is true and will notify you of UFHealth patient privacy policy. Read through the terms of use and accept for access.

After completion, you should be signed into your personal MyUFHealth page. If you have any questions or concerns, please call (352)294-5000 for assistance.

To Request an Activation Code

If you have lost your pre-visit summary or want to sign up before an upcoming appointment, visit https://mychart.shands.org/mychartprd/. Click on “Request an Activation Code”.  Provide your first and last name, medical record number (if you know it), UFHealth Hospital, date of birth, home phone, and email address. Submit the request to have your activation code mailed to your home.
